Interestingly enough but not surprisingly, at the top it says:

ਹੁਕਮਨਾਮਾ ਮਾਤਾ ਸਾਹਿਬ ਦੇਵੀ ਜੀ - Hukamnama Mata Sahib Devi Ji

 

Odd huh, where did the "Kaur" suffix come from...
