I guess they probably see it as an indulgence, and therefore something that demeans their spirituality. By spurning degh they signal their superiority to the rabble who are busy yamming it into their gobs as fast as possible. Just my opinion. ?

That might be another reason?

A lot of people avoid khandh these days.  

Not all of these people refusing degh is for medical reasons. Yes a small amount may not have it for diabetes etc.

If it is after Ardaas, and it’s being given out to all Sangat but some of the Sangat already had some earlier, I’ve been told second serving  should be declined

Don't know if any Rehit to support this though

I am referring to those who refuse degh all the time. But I can understand what you’re saying about refusing second serving
