the health aspect of a Sikh smoking should be 2nd, firstly is Guru Sahibs hukam on tobacco. No-one these days lights up not knowing that he/she has a cancer stick in his mouth.

I have, to my horror, seen indian girls smoking in india.

maybe he is a really sad and depressed person who has turned to smoking to heal himself and is fed up of life and doesnt care what the world thinks but at the same time he cannot cut his hair because of his love for sikhi.

btw obviously dealing with it the wrong way but just thinking that might be the reason.
